All youth organizations visiting the Hazel Outdoor Discovery Center must abide by the following guidelines to the best of their ability:

All visitors to the Hazel Outdoor Discovery Center are required to submit the signed release form. Youth organization leaders are asked to print, sign, scan (or photograph) and return the form to [email protected] before your group arrives.
